School Enrollment Packet
2026–2027 School Year

I, the undersigned parent or legal guardian, acknowledge that my child will be participating in activities at The Canopy School, which may include outdoor play, gardening, climbing, art, music, nature exploration, and other hands-on learning activities. 

I understand that participation in these activities involves certain inherent risks, including but not limited to falls, minor injuries, contact with natural elements, and other unforeseen incidents that may occur during normal school activities. 

By signing this agreement, I voluntarily assume all reasonable risks associated with my child's participation in school activities. 

I agree to release, waive, and hold harmless The Canopy School, its directors, employees, teachers, volunteers, and representatives from any liability, claims, demands, damages, or causes of action that may arise from my child's participation in school programs, except in cases of gross negligence or intentional misconduct. 

I confirm that my child is physically able to participate in school activities and that I have disclosed any relevant medical conditions, allergies, or special needs to the school. 

I authorize The Canopy School staff to seek emergency medical treatment for my child if necessary. I understand that every effort will be made to contact me or the emergency contacts listed on my child’s enrollment forms. 

I acknowledge that The Canopy School may include outdoor and nature-based learning experiences and that reasonable precautions will be taken to maintain a safe environment. 

By signing below, I acknowledge that I have read and understand this liability waiver and voluntarily agree to its terms.
